Community Champion
Job Summary:
This is for a full time position of approximately 24 hours committed to being the community champion and 16 hours of marketing per week. This position will serve as a liaison between the National Rural Health Resource Center, WCHNH, and community partners to improve local health outcomes. Assist with ongoing community health needs assessments and monitoring of changes in community health outcomes. Review, understand, and present the community health status report. Assist with planning activities, schedule Community Care Coordination events and workshops. Build awareness of the facility services. Assist with development and implementation of a Community Care Coordination plan and action plans.
Preferred:
•Bachelor’s degree or three years of experience in healthcare, communication education, or related field.
Skills:
•Excellent written, analytical, and oral communication skills. Public speaking experience preferred
•Three years of experience with community engagement and outreach
•Must be proficient in Microsoft Office, Word, Excel, and PowerPoint
•Project Management Experience
